To Order a Custom SSP Calendar: 
  1. Print out this order form.
  2. Fill out your personal information.
  3. Browse through the Dave Zortman Photo Gallery to select the 12 photos you wish to use.
  4. Write down the file numbers of the photos under each month you wish to use.  (You can drop the letters ssp in the file names)
    (File number appears when you place your mouse cursor over the photo.)
  5. Select the month and year you wish the calendar to begin with.
  6. Mail your order and check or money order to:
